This page explains what information is collected when you submit the “Chat to get matched” form and how it’s used.
Reminder: Submitting this form is NOT an appointment booking. Appointments are scheduled through Paul Mitchell Guest Services.
What data is collected
When you submit the form, we may collect:
- Name
- Phone number and/or Instagram handle (at least one is required)
- Requested service (optional)
- Availability (optional)
- Notes you provide (optional), such as your current set/removal needs and inspo details
- Preferred contact method (optional)
Purpose
This information is used only to match you to the right service and enable Ashlee to follow up with you about next steps, including what to request when booking through the clinic.
Not a clinic booking
Submitting the form does not schedule an appointment. To schedule, call (435) 752-3599 or book online through the Paul Mitchell Logan website, and request Ashlee Christensen by name.
Data deletion
If you would like your submitted information deleted, request deletion via Instagram DM: @ashes_nails_.
